The goal of Performance Physical Therapy & Sports Medicine inc. is to provide the type of service the staff would expect to receive if they were patients. Performance Physical Therapy is a Medicare Certified Rehab Agency. All major insurance are accepted. Private pay patients are accepted as well.
Performance Physical Therapy & Sports Medicine Inc. therapists are available: 8 AM – 6:30 PM Monday - Friday. Early morning, evening and Saturday appointments are available by special arrangement. Under Connecticut law you no longer need a treatment order for physical therapy before calling us. However, your health plan may still require it. Call us for details about your coverage.

DiPasquale with offices in both Hebron and Colchester.
In consultation with the professional staff, Chris continues to own and operate the PT practice.
